Yes, the [Add New Case] intake form has flexibility to add/edit/delete fields but only network administrators have the permissions to access these settings.

If you are NOT a network administrator:

  • Click the [Contact Admin] button in top-right corner of system to request changes to from your network administrator(s):

  • Your message may something like this:

    Subject: Request for new intake fields

    Message: Hello! Our agency is needing some more fields. Can we request the following be added?

    *Residential Status* (drop-down menu)
    - Homeless

    - Rent
    - Own

    *How did you hear about us?* (text field)

    Thank you!
